Change Management - Alarm System

control room

The Esso gas plant at Longford in South Eastern Victoria, until 1998, was responsible for supply of all the gas to Melbourne, the second largest city in Australia.

On the 25th of September 1998 Gas Plant One at Longford exploded, causing the death of two operators and injuries to eight others.

A daily event at the site was the constant unending number of alarms. There was no distinction between critical and non-critical alarms.

"Why were they operating an alarm mode like this?" Professor Andrew Hopkins poses this question in this safety training video.
