In this safety training video; behind the scenes, investigators found a range of issues involving the cut-price nature of the business, with outsourcing of most tasks, including heavy maintenance, and poor surveillance of quality by both the airline and the Federal Aviation Administration (FAA). The fatal crash investigation also uncovered an FAA memo recommending that Valuejet be grounded.

The FAA was compromised by having two tasks that were at odds with each other: they were supposed to regulate the industry, as well as promote the industry. On one hand, supposed to be corporate watchdogs, but also ensure all assistance was rendered in the promotion of the industry. Disturbingly, some indications were that mismanagement of key information was deliberate.
